Complete the claim form by following the instructions below (and on page two of the claim form). Then sign and return the original copy to our office and keep the yellow copy for your file. Incomplete or unsigned claim forms will be returned.

The "Order for Service Number" can be found at the top right hand corner of your Bill of Lading.

Include mailing and/or physical addresses with zip codes and a telephone number where you can be reached during regular business hours.

Complete pick-up and delivery dates, as well as warehouse information and who paid for your move.

Valuation type can also be found on the Bill of Lading. Remember that this is the protection you or your employer selected for your shipment and the means we will use to settle your claim.

Complete all columns for each article claimed:

Inventory number – we must have this number to be able to cross-reference this item to your shipment.

Article Weight – if unsure, please estimate.

Article Description – give a brief description of the item. If possible, use the same description used on the Inventory Sheets.

Description of loss/damage – describe the extent, location and nature of the damages; or if loss, give description of item.

Date of purchase/age of item – if unsure, estimate. To be considered an antique, the item must be more than 100 years old.

Cost to replace – indicate the article’s replacement cost today for same or similar item.

Amount claimed – enter the amount you are claiming, if amount is for repairs please estimate or write "repairs."

Carton damage – please indicate whether the carton was delivered damaged.

Please note the special instructions regarding computer, electronic and electrical equipment and appliances, this also includes Grandfather Clocks. (Inherent Vice)

If additional space is required, please be sure attached pages include the same required information.

You have nine months from the date of delivery to submit your claim. (Military Re-engineering Program Participants have 90 days to file their claim.)

Retain the damaged articles, including shipping cartons. These items must be available for inspection at the delivery location. Items that are not available at the delivery location may be denied.

Do not have any items repaired unless we advise you to do so.

All claims must be supported by paid transportation charges.

Your claim cannot be accepted unless all items are properly completed.

It is very important that you list all damaged items at this time -- No additional claim for items will be accepted after this report.
